Terms and Conditions:

• Cancellation must be presented in writing with a 30-day notice otherwise the monthly fee will be processed. A $300 cancellation fee will apply unless the term completed. If the membership is cancelled prior to the 12 months, any Free Treatments or Units that you have received will be charged to your credit card at full price.
• Fee Charged unless term completed or unless proof is provided of move 50+ outside of zip code where the office is located.
• Membership Fees are Non-Refundable.
• This membership fee recurs monthly unless cancelled prior to end of term. For membership renewal after 1 year, you will be charged $398 re-enrollment free, plus $149 for the first month, followed by 11 monthly recurring payments of $149.
• A minimum 12-month commitment is required. If membership is cancelled before the end of the 12-month term, you will be required to pay for any Botox/Dysport utilized but not paid for at the unit rate of $15. Any free A Botox/Dysport treatment and one free service is earned every 3 months after your enrollment date. INSIDER membership is only available online and not offered in the office. Additional units of Botox/Dysport on day of service are available at a special rate of $9.50 per unit. Memberships and membership discounts are non-transferrable and non-shareable and may not be combined with other office discounts or promotions. To receive discounted pricing online during the INSIDER membership term, you must be logged into your store account and follow checkout instructions.
